To detect an overdose, to determine liver damage and to monitor the effectiveness of treatment with an antidote.
When there are signs and symptoms of paracetamol toxicity (like nausea, vomiting, and abdominal pain).
The test for acetaminophen is used to diagnose overdosage of acetaminophen, liver damage, and to decide the need for treatment. As high levels of acetaminophen are hepatotoxic, other tests such as AST, ALT, PT are done to detect liver damage. Other tests like blood gases, lactate level, and metabolic panel along with creatinine are done to monitor the severity. Human chorionic gonadotropin (hCG) tests are also done in pregnant women to detect if the drug has crossed the placenta and harmed the fetus.
1) Is acetaminophen dangerous?
Acetaminophen is the safe and effective drug if used at recommended dose and interval.
2) How is acetaminophen overdose treated?
An antidote (N-acetylcysteine or NAC) is most effective if given within 8-12 hours following ingestion and symptomatic treatment. In few cases activated charcoal and gastric lavage may be needed. Severe cases of liver failure may require a liver transplant.
