The sex hormone binding globulin (SHBG) test is used to evaluate low testosterone levels in men and excess testosterone production in women. SHBG is a protein which binds to the testosterone, dihydrotestosterone (DHT) and estradiol hormone levels. Changes in the level of SHBG will affect the amount of hormone that is used by the body. Other tests like albumin level, prolactin, estradiol, and luteinizing hormone, may be done to evaluate if there is a balance of hormones in the body. In men, SHBG and total testosterone levels may be done to evaluate infertility, a decreased libido or erectile dysfunction. Measurement of SHBG helps to assess bioavailable testosterone with total testosterone levels. Increased SHBG in men may be associated with symptoms of hypogonadism as less free testosterone is available. In women, SHBG levels are important to evaluate the testosterone levels especially in cases of PCOS.
